The online forms will collect basic contact information about the youth participant, their parent/guardian, and emergency contacts. You will also need to complete a health history for the participant, and read and agree to risk and liability language required by Penn State Youth Programs. 

Before you begin:

• gather any contact information needed (emergency contacts, physician, etc.)
• make a note of youth's most recent tetanus immunization, dates of surgeries or hospitalizations, etc.
• be prepared to share information about the youth's allergies or medications

Once you begin the online forms, you will be entering the youth participant's information and answering questions about that participant. Once you complete the forms, please proceed to the checkout where you will enter your name/information in the "payment" field.

*Please note NO PAYMENT IS REQUIRED/WILL BE CHARGED. You will simply need to checkout with a "zero" cost.

Once you complete checkout, you will receive an automatic email to the address provided. If you do not receive this email within 15 minutes, please resubmit the forms. Often, the final "checkout" step is missed, so be sure to complete all steps to avoid having to re-enter information.

The confirmation email you receive will include pdf links of all the risk/liability language signed - please refer to those documents as needed.
